Peyronie’s disease gradually develops over a 12 to 18-month period in two specific phases. This is when most of the changes in the penis occur. Plaques begin to form, causing changes in the shape of the erect penis.
In the acute stage, plaque starts forming within the penis and increases in size and develops in shape. During this stage, the plaque will be softer and pliable. In the early months of the acute phase, symptoms may not be noticed but will nonetheless develop.
Patients will experience some pain or discomfort, a bending of the penis, and often a hardening or lump in one area. Most conventional treatments are not recommended at this stage, so if you are going through this you may have to grit your teeth and wait until the problem gets worse.
However, Focused Shockwave Therapy with its ability to break down plaque, is an excellent solution to prevent the condition from getting more severe. It should also overcome the onset of erectile dysfunction or the inability to have intercourse, because of a too severe deformity.
During the Chronic stage the plaque is fully formed and hardened, and pain has normally disappeared. The blood vessels around the afflicted area are clogged up with minute plaques and the penis will bend on erection. In most cases, the patient will suffer partial erectile dysfunction, a loss of girth and length, and a softening at the end of the penis.
Historically, the only treatment at this stage has been life-changing surgery. This might be for Penile Plication a shortening of the penis of up to 2 inches to straighten it, or the insertion of penile implants.
Penile implants rely on a manual pump to achieve an erection and many patients report a reduction on length as various prostheses are used.

Characterized by a bend or deformity in the penis, Peyronie’s disease in teenagers or young men can have a serious psychological impact as well as sexual problems.
While it is difficult to cope with this condition at any age, it is particularly devasting for a teenager who is at the early stage of maturity. Boys tend to be much more withdrawn than girls when it comes to coming forward about their medical problems and with teenagers the condition is compounded even further: How on earth do you tell your mum or dad or girlfriend that you have a bent or deformed penis?
Peyronie’s disease in teenagers often occurs as a result of an injury. A painful ‘kick in the goolies’ at school, a boisterous tackle on the rugby pitch, trying to hurdle a gate that goes wrong, or over-vigorous masturbation, can all be the trigger. There are very few men that have gone through life without feeling the excruciating agony in the private regions following an accident, whilst their friends gleefully laugh at their misfortune.
Very little research has been undertaken into Peyronie’s disease and teenagers. One study looked at 32 teenagers aged 15 to 19 versus men aged 40 plus, over a 10-year period to see whether there were any defining or differentiating factors between the two groups. The study found that 22% of the teenagers with Peyronie’s disease experienced painful erections and 37% subsequently developed ED.
Peyronie’s disease is characterized by a non-cancerous fibrous plaque that stops the penis from expanding on one side. In older men, it is normal that there is only one plaque. With teenagers’ there was a 700% increase in multiple plaques.94% of patients reporting a high level of stress and over a third had already sought medical help for anxiety and mood disorder and 28% had had a negative encounter with a sexual partner.
We advise any teenager reading this to share their condition with their parents and doctors and if suffering from depression, to seek counseling.

Benign Prostatic Hyperplasia- MANSMATTERS
BPH occurs when a man's prostate gland becomes enlarged and this is primarily due to sexual aging. The prostate gland plays a pivotal role in sperm production and as men age, the requirement for this becomes less.
In addition to the sexual function, the prostate also plays a valuable role in carrying urine from the bladder through to the penis. The result of sexual aging is that the prostate gland becomes enlarged and not only presses on the bladder but also on the pelvic floor muscles narrowing the tube that runs through it for a man to be able to urinate.
This triggers several effects, which catch up with men over time. One of the effects is an increased need to go to the loo. This is due to the prostate pressing on the bladder and telling the body it is time to go to the toilet.
Another effect is the narrowing of the tube that runs from the bladder, through the Prostate (the Urethra) resulting in it taking longer to urinate. And a further impact is that it takes more time to finish and post-finish there can then be dribbling.
The cumulative effect is that sufferers increasingly feel the urge to go to the loo, when they get there it may take some time to get started and significantly longer to finish. Plus, when they have finished, they have to be really careful because there may be dribbling that they have to manage.
It is normal that men who are diagnosed with BPH will be concerned about Prostate Cancer. There is, however, no increased link to Prostate Cancer.

Smoking can affect a number of bodily functions and lead to a number of complications. As a corollary, these complications can be strong contributory factors in the development of Peyronie’s disease. Some of these contributory factors are listed below.
Constricted Blood Vessels
Smoking can restrict blood vessels and cause them to narrow. This increases your chances of both a heart attack and high blood pressure. Healthy blood vessels are an essential part of achieving and gaining erections and the prevention of Peyronie’s disease.
Heart Disease
Smoking is a long-established factor in heart disease. It affects both those that smoke and those that are subject to smoke exhaled by others. Men with heart disease are prescribed Beta-blockers, which are a known contributory factor in Peyronie’s disease.
High Cholesterol
There is both good and bad cholesterol. Smoking will decrease good cholesterol known as HDL and increase bad cholesterol, known as LDL. It also increases triglycerides, which are fats in the blood. High Blood Pressure
High Blood pressure can gradually damage the linings of your arteries and will accelerate vascular disease (a degeneration of the health of blood vessels, arteries, and veins). Unhealthy blood vessels, that cannot function properly can lead to an increase of fibrous plaques and Peyronie’s disease.
Diabetes

The first sign a patient has Peyronie’s disease is during an erection when their penis bends in a different direction from normal. This is caused because plaque or scar tissue has formed in part of the penis, stopping it from expanding in that area. As a result, the penis bends in the direction of the plaque or scar tissues, whilst the other sides of the penis expand as normal.
The penis curvature can be upwards, downwards, to the side, or occasionally around the circumference. A patient may also notice a physical lump or hardening of the skin in the afflicted area of the penis. However, this is not always the case and a consultant can detect the plaque using diagnostic ultrasound.
Many men are too embarrassed to discuss their penis problems with a doctor, especially a female one, and allow their condition to progress further without treatment. Over time the condition can get worse leading to increased penis curvature as the plaque inside the penis increases.
Another problem then arises as blood flow through the blood vessels decreases leading to partial erectile dysfunction. Some, but not all patients will also experience different levels of penis pain. Sexual intercourse becomes much more difficult due to both the bend and the softening of the penis, reducing both the girth and the length.
A Check List of Peyronie’s Symptoms

Bump, lump or hard section on side of penis
Bent or curved penis in any direction
Pain within the penis upon erection
The decrease in penile girth and length

So these are the main symptoms of Peyronies disease.
Hard penis near the girth but much softer near the head
The severity of a bent penis depends on where the plaque is formed and its size and shape. Some patients may experience a slight or no bend but suffer a greater degree of erectile dysfunction.
Whereas Peyronie’s disease is indiscriminate, affecting men of all ages, certain patients are more prone to a build-up of plaque. This includes Type 1 diabetics; those with high blood pressure or high cholesterol; patients with high triglycerides; or a type of fat (lipid) in the blood.
Smoking or tobacco usage can also be a contributing factor. Peyronie’s disease can also be caused by trauma ranging from the bending of the penis during sexual intercourse, through to being hit in the most delicate of areas by a football.
Treating Peyronie’s Disease
Prior to the availability of Extra Corporeal Focused Shockwave Therapy patients were faced with a choice of dramatic invasive surgery. These included penile implants, where the patient ceases to have a natural erection and relies on a manual pump, and penile plication where the unaffected areas of the penis are reduced in length to straighten the penis. Men suffering an autoimmune disease are highly co-morbid to having Peyronie’s disease. An autoimmune disease is a condition in which your immune system attacks healthy cells by mistake.
In normal situations, the immune system is the body’s way of protecting itself from infection and it can tell the difference between good and foreign cells but with an autoimmune disease, a series of fighter cells attack the good cells. It releases proteins called autoantibodies that attack the healthy cells.
There are more than 80 autoimmune diseases, some like type 1 diabetes attack just one organ: the pancreas, whereas others like Systemic Lupus Erythematosus, attack the whole body.
Men who have an autoimmune disease are vulnerable to developing Peyronie’s disease if the immune system should mistakenly attack the healthy cells within the penis. This can lead to inflammation in the penis and can cause scarring.
Abstract
Introduction: Although heavily investigated over the last decades, Peyronie's disease (PD) pathogenesis remains unclear.
Aim: We sought to investigate the association between PD and autoimmune diseases (ADs) in men seeking medical help for sexual dysfunction in the real-life setting.
Methods: Complete sociodemographic and clinical data from a homogenous cohort of 1,140 consecutive Caucasian-European men were analyzed. Health-significant comorbidities were scored with the Charlson Comorbidity Index and ADs were stratified according to International Classification of Diseases, Ninth Revision classification.
Main outcome measures: Descriptive statistics and multivariate logistic regression models tested the association between ADs and PD.
Conclusions: Our observational findings showed that ADs are highly comorbid with PD in a large cohort of same-race individuals seeking medical help for sexual dysfunction in the real-life setting.

Peyronie’s Disease and Injury | How Small Injury Causes Peyronie's Disease | Mansmatters
In Peyronie's disease, when the penis becomes erect, the region with the scar tissue doesn't stretch, and the penis bends or becomes disfigured and possibly painful. A small injury to the penis can trigger Peyronie’s disease in men. Many men might not realize they have been injured at all. You could be hit by a cricket ball in the private parts or incur a small injury looking after a child in a playground. What might seem an innocuous accident, like being hit by a swing or being over-ambitious on a climbing frame, may become a life-changing event; e especially for the silent majority, who are too embarrassed to seek medical help.
With middle-aged to older men, Peyronie’s disease is often caused by injury during sexual intercourse. As we age our ability to get hard erections decreases. Our bodies' ability to channel up to 20 times as much blood into the penis decline and the blood vessels that accommodate this blood become less effective.
This means that as we get older, many men have sexual intercourse with a less rigid penis, making it far more vulnerable to being accidentally bent. Accidental bending of the penis during sex can lead to internal trauma, with microscopic bleeding and damage to the tissue. This leads to scaring and to onset of Peyronie’s disease.
Whilst it is a highly sensitive subject, we do urge men to discuss this with their partners. There are some measures, which will significantly decrease the risk of injury and subsequent onset of Peyronie’s disease. This is also important for men that have been cured of Peyronie’s disease through focused shockwave therapy to prevent a reoccurrence.
* Speak with your partner so they are aware of the risk and be a little less free-spirited.
* Consider taking freely available medication like Viagra, Cialis, and Levitra. You may feel you don’t need it, but it will help prevent injury.
* Discuss with your partner whether vaginal lubrication would help. This can be purchased at any chemist.
* If your manhood slips out during intercourse take care of re-entering. Be overcautious, rather than over-zealous.
* Reconsider sexual positions. The most vulnerable position for a man is when their female partner is on top of them. Injury is extremely likely and if you have already suffered Peyronie’s Disease you are likely to trigger it again.

There is a direct correlation in both conditions, and both can be treated with focused shockwave therapy.
In order for a male to have an erection, in the region of 20 times as much blood as normal needs to swiftly flow and be held within the penis. The increased blood flow swells up the penis until it becomes hard and rigid.
With both Peyronie’s Disease and ED, plaque forms in the blood vessels impeding this flow and preventing a full and satisfying erection.
With Peyronie’s Disease, this is exacerbated and becomes more pronounced with a lump of plaque generally forming on the side of the penis. As a result, this side of the penis cannot expand as normal and when an erection takes place it results in a bend in the penis.

Additional Treatment for ED Sufferers
Peyronie’s Disease patients who have also suffered from moderate to severe ED previously may also be prescribed a treatment of shockwave therapy for their ED.
This has two main benefits. The first is in increasing and revitalizing the blood vessels within the penis. This is known as angiogenesis. More blood vessels allow more blood to flow into the penis and enable a stronger erection.

Peyronie's disease can cause stress, anxiety, and depression in men. Some studies show over half of the men diagnosed with Peyronie's will suffer from depression. If not diagnosed early, the disease can cause problems with erections and lead to less satisfying sexual relations.
Some men suffer depression because of a severe medical condition and others because of a life-changing experience. To many men, the development of pain and the shock of increased curvature in their penis is extremely distressing. This compounded with symptoms of erectile dysfunction and the struggle to have satisfactory sexual intercourses, leads to diminished confidence leading to depression. In one study 48% of men with Peyronie’s disease were diagnosed with depression, with 21% being at the severe end of the spectrum.
From our clinical experience, Peyronie’s disease depression can be avoided by many men, if they sought help early on and all available treatment options fully explored.
In the vast majority of cases, men suffer in silence as the bend of their penis increases. Some men avoid having sex or talking with their partners, leading to psychosexual and relationship breakdown. It is not rare for a patient to show emotional distress when they first visit our clinic or express eternal gratitude after successful treatment. In some instances, it is necessary for us to combine shockwave therapy with a psychosexual counselor.
Men who seek treatment have often faced an uphill battle. At a GP level, they may be prescribed topical treatment for pains and eventually referred to a urologist. This often occurs over a long time period during which depression and fear have the chance to set in.
