(ISBN 0929385225) Written after Cooper had been a member of the US Naval Intelligence Briefing Team of the Commander in Chief of the United States Pacific Fleet, the book details many of Cooper's claims about the alleged influence of UFOs on US government agencies, and the New World Order, the assassination of President John F. Kennedy. Another claim made in the book pertains to his own death. William Cooper, seemingly paranoid, claimed that he would be shot to death on his own property. True to his "fate," he was gunned down in precisely this manner years after publishing the book and settling down in Arizona. The title of the book alludes to a passage in the Bible, Revelation 6:8: "And I looked, and behold a pale horse: and his name that sat on him was Death, and Hell followed with him." The circumstances surrounding Cooper's death are controversial. On November 5, 2001, officers of the Apache County Sheriff's Office decided to serve Cooper a warrant based on the above mentioned charges. For reasons not explained, the Sheriff's Office sent deputies to Cooper's home at approximately 11PM, and instead of knocking on his door and announcing themselves, the deputies attempted to lure Cooper from his house by posing as civilians playing loud music on or near his property. Cooper, who was an above the knee amputee, went down to investigate the scene in his truck. It is disputed whether the deputies identified themselves or tried to serve an arrest warrant at that time. Nevertheless, Cooper announced he was returning to his house to contact the Eager Police Department, either to verify the warrant or to contact the local authorities in order to report what he may have still believed was a real disturbance. Before he could do so however, the sheriff's deputies tried to apprehend him, at which point shooting began. Although it is unclear who began firing, there are some who question the uncertain circumstances, and who was responsible for first shots fired. Independent investigators questioned who would fire first at a double amputee, and Cooper's supporters claimed it was a convenient death, given the close proximity to the September 11th attacks, of which Cooper had already spoken out, with allegations that subsequently would find voice with the large 9/11 Truth Movement. Cooper was armed and one deputy was wounded. Another deputy returned fire, killing Cooper. Milton William Cooper was 58 years old.

The book contained Martin's views on the Jewish question in Europe and on the Second Vatican Council. Journalist Joseph Roddy alleged — in a 1966 Look Magazine article about the debate about Jews during the Second Vatican Council — that one and the same person under three different pseudonyms had written or acted on behalf of Jewish interest groups, such as the American Jewish Committee, to influence the outcome of the debates. Roddy wrote that two timely and remunerated 1965 articles were penned under the pseudonym F.E. Cartus, one for Harper's Magazine and one for the American Jewish Committee's magazine Commentary. Roddy alleged that tidbits of information were leaked to the New York press that detailed Council failings vis a vis Jews under the pseudonym of Pushkin. Roddy claimed two unidentified persons were one and the same person — a "young cleric-turned-journalist" and a "Jesuit of Irish descent working for Cardinal Bea...who was active in the Biblical Institute" — he figuratively named as Timothy O'Boyle-Fitzharris, S.J. so as not to reveal the true identity of his source. In his 2007 book Spiritual Radical: Abraham Joshua Heschel in America, Edward K. Kaplan confirmed that Martin cooperated with the American Jewish Committee during the Council "for a mixture of motives, both lofty and ignoble...[He] primarily advised the committee on theological issues, but he also provided logistical intelligence and copies of restricted documents." It is confirmed in the book that Martin used the pseudonyms Forest and Pushkin. Kaplan acknowledges that The Pilgrim by Michael Serafian, was requested from Martin by Abraham J. Heschel, who arranged for the book to be published by Roger W. Straus, Jr.'s Farrar, Straus and Giroux printing company. It was published in the hope that it would influence the deliberations in the council. Once Martin's identity as author was revealed, it led to protests "and the book had to be removed from circulation at considerable financial loss to the publisher". Kaplan lastly states that Martin was the primary source of information for Joseph Roddy in writing his 1966 article for Look Magazine, and that O'Boyle-Fitzharris was, in fact, Martin. Kaplan judges the Roddy article as "dangerously misleading [due] to the credence it gives to the claim that without organised Jewish pressure the council declaration on the Jews would not have been accepted." Martin explicitly denied he was a spy, along with denying other rumors. Michael Cuneo, in his book American Exorcism, writes, "Martin told me that he was perplexed, and more than a little annoyed, by the swirl of rumors surrounding his personal life."
