【106】PCR検査に依存している限り騒動は終わらない - 大橋眞

PCR依存症とも言えるコロナ騒動ですが、PCR検査の問題点を理解していない人が多いようです。PCR検査の充実が感染拡大を防ぐ対策と考えて、地方自治体独自でPCR検査体制を拡充するところが出てきました。PCR陽性者の増加が、感染症の広がりを意味していると考えているためだと思います。咽頭スワッブ検体中の新コロウイルスは、インフルの場合と比較すると、極めて少ないのです。ウイルス数では1万分の1程度でしょうか。インフルの場合、症状が出ているときは、10億コピーに達するほどウイルスが増加します。新コロウイルスは、インフルの1億分の1程度とします。この場合医学的に意味のあるのは、インフルウイルスだけで、新コロは全く考えなくても良いはずです。伝播するのはインフルだけで、新コロは伝播しないのです。症状にも新コロは全く関係ありません。しかし、この時にPCR検査をしたら、どうなるでしょうか。
　PCRは、プライマーが新コロ遺伝子を増やすように設計されているので、新コロ遺伝子だけを増幅します。PCRで新コロ遺伝子が増えて、インフルと同程度のコピー数になります。しかし、PCR検査の結果は、新コロ陽性という結果になり、インフルの方は全く結果に反映されません。
　もともと少数の新コロウイルス（変異体）を多くの人が持っている状態であろうということは、このウイルスが様々な動物や植物などに分布していることから想像できます。これまで医学的に重要なウイルスではなかったために、調べていなかっただけです。インフルのように多数のウイルスを持った人が次々と見つかるような状況であれば、急速な感染拡大もあるかもしれません。しかし、PCR陽性であっても少数のウイルスしか見つからない人が大部分であれば、急速な感染拡大でなく、もともと持っていた常在ウイルスがPCRに反応しただけということが判ります。PCR検査の結果を陽性者の数で表すのは意味がありません。PCR検査のサイクル数を現在の半分にすれば、10万コピー以上の人を陽性とすることもできます。これなら、ある程度医学的に意味のある検査になるかもしれません。陽性者の数も激減して新コロ騒動は終結すると思います。

However, if most people are PCR-positive but only a few viruses are found, it is clear that the original resident virus has just responded to the PCR, not the rapid spread of the infection. It makes no sense to represent the results of a PCR test in terms of the number of positives. By halving the current number of PCR test cycles, more than 100,000 copies can be positive. This may be a medically meaningful test to some extent. I think that the number of positive people will decrease sharply and the new roller turmoil will end.
